There are four files below which MUST be downloaded for all accepted members into the All-State Jazz Band. Two of the files are forms which must be completed and mailed before September 15th. If the school is paying fees for the student, these forms must be mailed in immediately.

These files are: Hotel Registration Form; Medical Form; Information Sheet; COJ Information for Parents.
